- Rationale
1Our general surgery service runs two teams, General Surgery A and B (Gen A and Gen B), each with 3 to 4 residents and 2 interns for 60 to 70 patients. In each team, a first-year resident and an intern cover the male ward and a second-year resident the female ward, each with medical students if any, while the third-year and fourth-year residents and an intern cover the private ward, consults, the surgical ICU and others.
2Our residents and interns also run the outpatient clinic, the operating room and consults whenever the day's activity needs them, and they work shifts; when an operation is set after hours, residents off shift come in to help. Gathering every patient's data before rounds and again before reporting to the attending meant the day started early and finished late.
3Medical students, interns and residents copied vital signs, labs and operative notes by hand from the old electronic medical record (EMR) into the shared Sheet and Trello cards.

8Gen A and Gen B already used Trello and the shared Sheet before AutoVS, and I improved both along with it.
9Our LINE group announces each admission or consult: admit or consult, the department and attending, then the patient's name, hospital number (HN), underlying disease, diagnosis and plan. For each general surgery admission, someone copied the notice from LINE, pasted it into a new Trello card and tagged the attending by hand.

11Now we just move each card from New Admission to the Gen A or Gen B board.

17The KPI I care about is efficiency and extra sleep for everyone in Gen A and Gen B: vital signs arrive on the Trello cards, and new admissions arrive as cards ready to move.
